Patient Satisfaction with Clear Aligner Therapy: Clinical Outcomes, Patient Experience, and Contemporary Evidence

Abstract

Clear aligner therapy (CAT) has transformed modern orthodontic treatment by offering an esthetic, comfortable, and removable alternative to conventional fixed appliances. Over the last two decades, advancements in digital orthodontics, three-dimensional treatment planning, computer-aided design/computer-aided manufacturing (CAD/CAM), and thermoplastic materials have significantly expanded the indications and effectiveness of clear aligners. Patient satisfaction has emerged as a critical outcome measure because treatment success is increasingly evaluated not only through clinical outcomes but also through patient experiences, quality of life, comfort, and psychosocial well-being. This review examines current evidence regarding patient satisfaction with clear aligner therapy, focusing on esthetics, comfort, oral health-related quality of life, speech adaptation, treatment effectiveness, compliance, and overall patient perceptions. Findings suggest that clear aligners generally achieve higher satisfaction scores than conventional fixed appliances due to improved appearance, reduced discomfort, enhanced oral hygiene, and greater lifestyle convenience. However, treatment success remains highly dependent on patient compliance and case selection. Future innovations involving artificial intelligence, smart aligners, and digital monitoring systems are expected to further improve patient experiences and treatment outcomes.
